The Patient Safety Division
The Patient Safety Division at the Tel Aviv Medical center is responsible for ensuring patient and healthcare-provider safety. The division’s multitiered program is aimed at achieving this goal. The division promotes both proactive and reactive processes at all organizational levels to establish a culture of quality and safety.
Patient Safety
- Investigation of safety events (reactive activity)
- Identification of danger zones and implementation of corrective actions (proactive activity)

Promotion of a culture of safety in the hospital
Risk Management
- Management of adverse events reports and claims
- Establishment of a continuous working relationship with the insurance company
- Legal counseling
Standardization and Audits
- Development of a regulatory medical framework
- Implementation of new organizational safety and security policies
- Routine audits
- Preparation of the organization for the accreditation process and ensuring sustainability of achievements
Medical Records
- Maintain state-of-the-art standards of medical record documentation
- Maintain quality of medical record archiving (including digital records)
- Develop and implement an operationally and economically efficient archiving infrastructure
HMO Audits
- Maintain a rational balance between quality of care and economical robusteness of medical care
- Resolving financial disputes between HMOs and the hospital
Israeli Ministry of Health Committees
- Maintenance of all organizational committees (composition, timely and ad hoc meetings)
- Managing the interface between hospital and Ministry of Health
